The pathway provides guidance from referral to the completion of therapeutic support. Clients will receive support throughout their recovery journey.
Step 01
Referral
We require the referrer to complete our referral form to enable us to understand the needs of the individual and to examine if our service can effectively meet those needs.
Step 02
Initial Baseline Assessment
Our baseline assessment entails information gathering which includes one to one clinical psychosocial assessment to gain a comprehensive understanding of the individuals mental, emotional, social and behavioural health needs.
Step 03
Formulation
Our clinical team will develop a holistic plan based on gathered information, ensuring alignment with client’s needs.
Step 04
Therapeutic Support Plan
Person centred therapeutic support plan that addresses emotional and mental health needs and supports the individual's well-being through trauma-informed practice. We take into consideration the clients preferences and goals including Advance Statements.
Step 05
Therapeutic Risk Assessment
Proactively manage risks through comprehensive assessments, including emotional, behavioural, and mental health aspects, with crisis and contingency plans for client safety and security.
Step 06
Review
Regularly reviewing and reassessing support and risk management plans to adapt to evolving needs and circumstances.
Step 07
Outcome Measures
Monitoring and measuring outcomes to gauge progress and effectiveness of therapeutic interventions (Evidence-Based Practice).
Step 08
Celebrating Success
Recognising and celebrating individual achievements and milestones reached during the recovery process.
Step 09
Offer Outreach House
Providing the option of transitioning to an outreach house for those ready to take steps towards increased independence ( without round-the-clock staffing).
Step 10
Transition to Independent Living
Facilitating a structured transition process for individuals moving towards independent living (Own Home).
Step 11
Discharge Summary
Compiling a comprehensive discharge summary to capture the individual's progress, achievements, and future recommendations.
