Our Care Package Intervention

We use the Life Balance Model to guide all our support. This model helps clients build daily routines and activities that promote health, well-being, meaningful engagement, and a strong sense of identity. Our goal is to support clients to live balanced, fulfilling lives within their community. Explore our model below

Our Care Pathway

The pathway provides guidance from referral to the completion of therapeutic support. Clients will receive support throughout their recovery journey.

1. Referral

We require the referrer to submit a referral form to enable us to understand the needs of the client and to examine if our service can effectively meet those needs.

3. Formulation

Before the client moves in, a therapist develops a detailed formulation using pre-assessment information, clinical insight, and input from the social worker, family, carer, or friends. This formulation helps the team understand the client’s needs in a psychologically informed way. Reviewed every 6 months and shared with the client and relevant professional.

2. Pre-Admissions Initial Baseline Assessment

We complete a person-centred, strength-based assessment to explore what matters to the individual, focusing on their abilities, needs, and the support that will help them thrive

4. Therapeutic Support Plan

Within six weeks of admission, we co-create a personalised Therapeutic Support Plan with the client and professionals involved. With consent, family, carers or friends may join. Reviewed every 4 to 6 weeks and shared with the client and relevant professional.

5. Risk Assessment & Crisis Contingency Plan

A risk assessment and crisis plan is completed with the client and professionals involved. With consent, family, carers or friends may also be included. It identifies triggers, warning signs, protective factors, and strategies to maintain safety. Reviewed every 4 to 6 weeks and shared with the client and relevant professionals.

6. Outcome Measure

To measure how well our care model intervention is working, we use trusted tools like the Camberwell Assessment of Need Short Appraisal Schedule (CANSAS). This is reviewed with the client and professionals involved. With the client’s consent, family, carers or friends may join. These tools help track progress and identify new needs,. Reviews take place every 6 months and shared with the client and relevant professionals.

7. Celebrating Success

We recognise and celebrate the client’s achievements, progress, and new skills gained. Highlighting strengths and successes boosts confidence and supports ongoing growth. With the client’s consent, these moments may be shared with family, carers, or friends, and can be featured in our service newsletter in a way that respects confidentiality.

8. End of Service Transition

Discharge readiness is agreed collaboratively, with the client at the centre. Outcome measures and the formulation document track progress from admission, showing the client’s baseline strengths and needs, and indicate when 24-hour support is no longer required. Usually when greater independence is demonstrated.

9. Discharge Summary

The therapist co-produces a discharge summary with the client, outlining progress, strengths, and ongoing support needs. This summary is shared with the client and relevant professionals to ensure a smooth transition. With the client’s consent, family, carers, or friends may also receive a copy.

10. Six Weeks Outreach Support Package

Clients will be offered a 6-week outreach support package to facilitate their transition into their new environment.

Our support workers partner with clients, rebuilding confidence and fostering independence through personalised care, empowering clients to independence.

Therapist

The clinician plays a key role in supporting clients and the team by developing therapeutic support and risk management plans. They ensure that clients are placed on the appropriate intervention tiers based on their needs.

Registered Mental Health Nurse​

Our in-house registered mental health nurse offers expert support, providing guidance, assessments, and interventions to enhance well-being, including safe medication management for holistic care.

Team Manager​

The manager oversees the efficient management and operation of the service. With extensive experience in mental health care and regulatory compliance, they provide leadership and guidance to ensure high-quality service delivery.
